Establishing trust

Medical Case Management is at the heart of Action Wellness. Our Medical Case Management program serves thousands of  individuals and families each year in more than 30 locations, and is recognized both regionally and nationally.

Establishing trust through one-on-one relationships, our Medical Case Managers work closely with clients to coordinate care, navigate through complex government systems, and expertly access medical, social, legal, and emergency services. We take great pride in the innovative programs we have developed and launched that uniquely address unmet client needs with measurable outcomes.

Our Medical Case Managers include specialists trained in providing supportive services for:

  • the LGBTQ community
  • deaf and hard of hearing
  • drug and alcohol addiction
  • families
  • immigrants
  • mental health
  • perinatal care
  • individuals in the prison system
  • women
  • youth

Our Case Managers are fluent in many languages including Spanish, French, Arabic, and American Sign Language.

The Medical Case Management program at Action Wellness also offers emergency services coverage 24 hours/7 days a week, referral services, direct emergency financial assistance, and more.
